We strive to ensure that all patients, visitors, clients and other members of the public have equal opportunity and access to the services provided at The University of Toledo Medical Center. Requests for accommodations should be arranged with the department scheduled to service the patient.

In the event your request is not granted, the individual has the right to utilize the complaint procedure as defined by policy. A Customer Care Center representative at Ext. 3606 can assist in obtaining the appropriate forms.

Advance Medical Directives - Stating your wishes regarding medical treatment
Today, many people are concerned about the medical care they would receive if they became too sick to make their wishes known. Some people may not want to spend months or years dependent on life support. Others may want every step taken to lengthen life. The University of Toledo Medical Center recognizes that each competent adult has the right to determine what his/her end of life treatment should be. This includes the right to execute Advance Medical Directives.

The Ohio State Law recognizes the Living Will, the Durable Power of Attorney for Health-care and the DNRCC (Do Not Resuscitate Comfort Care). To obtain forms to complete these directives, call Admitting at 419.383.3881 or see an admitting representative located in the hospital lobby.

Electrical Equipment
For your safety, our staff must approve all personal electrical equipment brought into the hospital for patient use. Please work with our nursing staff to coordinate this process.

Interpreter Services
If you require an interpreter to understand hospital procedures, diagnosis, condition or treatment, UTMC provides this as a complimentary service. Contact the Customer Care Center at 419.383.3606 to request assistance.

Complimentary assistance devices are also available to patients free of charge by contacting Pastoral Care at 419.383.3851 for battery-operated units to enhance human voice.

Medications
Please bring a list of medicines you are currently taking, including the dosage and frequency of each.

If you bring medications with you, the staff will take note of them and ask you to send them home with a family member or other designated person. Your physician will prescribe the medications you need during your hospital stay.
