Before You Are Admitted

Pre-Arrival/Pre-Registration

The University of Toledo Medical Center would like to make it as easy as possible to register for your admission or outpatient procedure. By Pre-Registering for your visit, you will be able to move through the registration process quickly and smoothly.

For your convenience, the Pre-Registration Department is open six days a week, Monday through Friday from 8 a.m. to 8 p.m. and on Saturday from 9 a.m. to 5 p.m. To take advantage of this service, please call (419) 383-6272, or toll free at (800)-321-8383, extension 6272.

The Pre-Registration Process:

  • Your doctor will contact registration and tell us about the service or procedure you need.
  • A Registration Interviewer will call you and ask a series of personal and financial questions. This information will be kept confidential.
  • During the Pre-Registration Interview, you will need to give your insurance information as well as facts about yourself, such as your address and date of birth. Providing the correct insurance information will help your bills get processed quickly and accurately. This information will allow us to give you an estimated cost of services that your insurance does not cover.
  • If you have not received a phone call from the Pre-Registration department within 2 days prior to your appointment, please call 419-383-3881 or outside of Toledo call 800-321-8383 ext. 3881.

When calling in, please have your health insurance card available. You will be asked specific questions regarding your health insurance. The process takes approximately five minutes.

When you arrive, please stop at the registration desk to have your insurance card copied and to sign consent forms.

Checking In

Each day a large number of patients with different medical needs and schedules come through the University of Toledo Medical Center. We will try to meet your needs during the admission process, but you may meet delays while waiting for your room. We regret any inconvenience.

When you arrive at the reception area please check in with the receptionist, giving your name and the name of the physician with whom you have the appointment.

Your Responsibilities

To expedite the check in process please be aware of the following:

  • Bring your photo ID.
  • Make sure you have your current insurance cards with you at all time.
  • Know your insurance benefits. It is your responsibility to know your insurance company's requirements and procedures.
  • Make sure your doctor and the University of Toledo Medical Center are listed as participating providers in your plan's provider book.
  • If your insurance requires a co-pay or out-of-pocket expense, payment will be expected at time of service.

Your Patient Identification Band

Each patient gets an identification (or "ID") band when checking into the hospital. The band contains important information about you that helps us meet your individual needs. Please wear the band throughout your stay. If your band is removed, falls off, or becomes uncomfortable, let your nurse know immediately.

Guest and Patient Information Desk

The Guest and Patient Information Desk is located in the lobby of the Medical Center. The desk is staffed with ambassadors who are happy to assist patients and guest with non-medical needs or questions. Call 419-383-3866.
