Occurrence Reporting
An occurrence report is used to document an unsafe condition or event when it occurs. Anyone who discovers, witnesses or is notified of an occurrence should complete an online occurrence report. Look for the Safety Net icon on all Health Science Center computers to complete safety event reports including unsafe conditions.
