Endocrinology Outpatient Services

The University of Toledo Outpatient Services Clinic specializes in the treatment of diabetes and many other Endocrine-related disorders.

The Outpatient Clinic has recently obtained accreditation as a certified diabetes education program by the American Association for Diabetes Educators (AADE) for the treatment of Type 1 and Type 2 diabetes. The education program will provide experience from Certified Diabetes Educators (CDE) and Registered Dieticians (RD) to help patients understand their diabetes, and treat themselves effectively.

Physician's Offices: You will need to provide our office with the following information before we will schedule an appointment:

• Reason for consultation - order for consultation signed by referring physician
• Demographic information
• Insurance information
• Medication list
• Labs, tests, scans, etc. related to appointment
