Cardiac Rehabilitation Phases

While a heart attack or other heart condition can have a major impact on how long you live and the quality of your life, taking care of your heart can dramatically reduce your risk of living a shortened and disabled life.

The cardiac rehabilitation program at The University of Toledo Medical Center helps patients with heart and vascular disease improve their health and continue on with their lives.

Phase 1:
Cardiac rehabilitation initiates in-house rehabilitation for hospitalized patients including supervised light exercise, initial dietary education, and personalized guidance regarding low-level home activity programs following hospital discharge. Patients are then referred to an outpatient phase 2 cardiac rehabilitation program near their home.

Phase 2:
Cardiac rehabilitation provides a medically monitored, personalized exercise training program coupled with scheduled health education classes and individual counseling for patients with demonstrated cardiovascular disease. Specific educational programs include nutritional and weight loss counseling, cholesterol management, diabetes education, smoking cessation, stress management, and
cardiac risk factor awareness training.

Phase 3:
Cardiac rehabilitation provides a supervised exercise training program and health education classes to patients with cardiovascular disease or significant cardiovascular risk factors in need of modification. Ongoing follow-up of diet, cholesterol levels, and body weight in addition to direction regarding maintenance of a home conditioning program is included in this phase.
